


Case Management
Families are able to connect with a Family Support Worker in a case management capacity. This can include emotional support in your parenting role, parenting education, referrals to relevant supports and services, advocacy on your behalf with external departments and agencies, sharing relevant resources around community support services and parenting, and connecting you with community groups for peer support and social connection. The Family Support Worker may visit you in your home, in the community, or at the Community Centre. They are also able to support you to attend appointments regarding your family. The Family support team strive to be culturally aware and inclusive in our practices with families, have some understanding of disabilities and impairments, and a safe space for the LGBTQIA+ community. We endeavour for continuous improvement in our learnings to best support the families we work with.
Once linked with the Family Support Program for case management, referrals can be made to relevant agencies to support family functioning and wellbeing. For example, the worker can assist you with referrals to external agencies for support with mental health, child health and development, financial assistance, domestic and family violence, social and community housing, and other relevant services depending on your family's needs and goals. The worker can complete the referral with you or on your behalf, can be there when you initially connect with the other agency, and can continue supporting you and your family around parenting.
The worker can also provide advocacy on your behalf with government departments and other agencies such as schools and child care, Centrelink and Department of Housing. This can include attending appointments with you, making contact on your behalf and providing support letters.
The Family Support Program has specific eligibility around supporting Parents and Carers with children under 18 years of age in their care. Our geographical area includes Sunshine Coast Hinterland, and the southern part of the Sunshine Coast. Please contact us to check your eligibility. If you are out of our catchment area, we will try our best to provide linkage with supports relevant in your area.
