Welcome families,
We look forward to walking alongside you on your parenting journey.
We would like to take this opportunity to welcome you to our Centre and warmly invite you to connect with the other programs and community groups which we host in our space.
Our approach as an organisation is to support people to strengthen their social support networks by building connections in the community.
Have a chat with your worker if you would like to know more about what is on offer at both of our community centres.
As a participant in our Family Support Program, you may like to know more about what to expect when you engage in case management with us.
Your allocated worker will meet with you to discuss your support needs and develop a plan to work towards your goals.
They will work together with you on goals which involve parenting skill
development and family functioning. They are also able to provide referrals to other supports/services where necessary, such as support to access mental health care. They can also assist with advocacy where needed with other services and departments such as housing, Centrelink, schools and child care.
Support needs are determined by you, you are not required to disclose any information or engage with any supports or services you do not wish to.
The level of support and contact you have with your worker is agreed upon between you and your worker, there are no formal timeframes for engagement.
However, as part of our funding agreement, we are not able to support families who do not have children under the age of 18.
All information you share with your worker is confidential. We will not share information with other services or any other parties without your consent to do so. However, if your worker believes there is a risk of harm to yourself or others, they are mandated to report this information.
Case notes are recorded by your worker of all family support interactions.
These case notes are stored on a secure online database. This information is not accessible outside of Caloundra Community Centre staff. You have the right to request to view your case notes.
Your participation is voluntary and you have the right to withdraw from case management at any time. You have the right to make a formal complaint to Caloundra Community Centre if you are unsatisfied with the conduct of your worker and/or the support you have received.
Please visit www.calcomm.org.au/policies for more information on our
policies, including Complaints, Mandatory Reporting and Confidentiality.
If you have any further questions or concerns, please have a chat with your worker.
We look forward to working with you!
